Financial Criteria
Families who have lower income
Eligibility
Children aged 21 or younger with a physical disability from families who have lower income. The foundation classifies special needs as those children under the age of 21 with any type of physical disability (such as but not limited to, spina bifida, paralysis, missing limbs) or illness (such as but not limited to, cerebral palsy, multiple sclerosis, cancer).

None specified but the Foundation will provide funds for devices which are necessary or improve the daily functioning of the child.
Application Process
Online applications are accepted at any time.
Requests are reviewed monthly. If your completed application (including ALL required support) is received up to a week before a voting meeting, it will be reviewed at that month’s meeting. If received within a week of a voting meeting, it will be reviewed at the following month’s meeting. Please note, applications will not be reviewed until ALL required support is received. Proof of the child's physical disability in the form of a letter from the child's physician, therapist, social worker, or school on official letterhead is required with the application, in addition to proof of insurance denial for any item requested, where applicable. Further documentation may be required at the discretion of the board. Proof of income in the form of a copy of your most recent tax return is required with the application. Further documentation may be required at the discretion of the Giving Angels Foundation.
Applicants must respond to the following question when requesting a device: Why will this item be necessary or improve the daily functioning of your child?
Please be as specific as possible. Please also provide detailed descriptions of or link(s) to the specific item(s) you are requesting. There is a $500 maximum, one-time grant allowance per family.
